We have executed many projects, small and large ones. Most of the time we’ve found that lot of defects raised by the testers are turned back by business analysts, system analysts, developers, etc citing the reason like working as designed (WAD), data error etc.

Such defects are termed as rejected defects. A high number of these defects is not a good sign for the testing team. I was leading one such critical project where there was a significantly high percentage of these rejected defects. This presentation emphasizes a critical area of defect prevention and management i.e. rejected defects and the process/methodology adopted in reducing the a percentage of rejected defects using the '6 Sigma' approach.

In the measure & analyze phase of the project, I used pareto analysis, cause & effect diagrams, control impact matrix to identify the root cause for such high percentage of rejected defects. After analysis, I came up with a improvement plan which was successfully implemented in the project and the results bore good fruits of this improvement plan in terms of the final output.  I implemented this project in 9 months i.e 3 quarters. Typically such projects need to be run for 3-4 quarters to show the desired output and results.
